About this event

The Summit will take place on November 2, 2023, at the We-Ko-Pa Conference Center in Ft. McDowell, AZ, just outside of Phoenix, Az. In order to address COVID health disparities, the summit aims to bring together tribal leaders, healthcare organizations serving American Indian/Alaska Natives and key stakeholders to create a path forward for diabetes solutions and health equity in Indian country.

Who should attend?

Healthcare professionals from Indian Health Services, Tribal Health (638 facilities) and Urban Indian Health Organizations, Diabetes Program Personnel, Healthcare Leaders, Tribal Nation Health Department/Programs, Community Health Representatives, Community Health professionals, Physicians, school nurses, pharmacists, other clinicians, health educators, community government personnel, other public health professionals interested in updating their knowledge and skills in Prevention and management of Diabetes and Health Equity.

Scholarships Awards

Limited scholarships are available to assist Tribal Health Organizations obtain DSME or NDPP certification status. The scholarship funds do not cover the entire cost of the entire certification process. There are two (2) types of scholarships available:

Organizational DSME Scholarship:

Up to 5 organizations shall be awarded the DSME Scholarship. Each Organization shall be awarded $1,100 to be used towards the initiation and initial fees for accreditation support. Ex. ADA/ADCES-accreditation application fee. This scholarship award is open to IHS, Urban and Tribal Health Diabetes Self-Management Education (DSMES) Programs/National Diabetes Prevention Programs (NDPP), and programs in areas of high need.

Individual NDPP Scholarship:

Up to 15 Healthcare professionals to be awarded. Each of the 15 selected healthcare professionals shall be awarded $649. The scholarship to be utilized on NDPP Lifestyle Change Coach training. Organizations may also apply on behalf of their program staff members.

Selection announcement shall be made at the 2023 Tribal Diabetes Health Equity Summit on November 2, 2023.
