What Care Do I Need and When?
Have you ever wanted to better understand different types of caregiving options, such as nursing care, home health care, types of care facilities and hospitals? It can be tough to know what everyone does and when they do it.
Join a panel of experts as we discuss transitions in care as we age. Our panel will include primary care, rehabilitation services, nursing facilities, assisted living facilities, in-home care providers as well as Palliative Care and Hospice.
Community partners will be on-hand to provide information on services, as well as refreshments and raffle prizes! This event is FREE to attend.
