Philadelphia Eagles Linebacker @NakobeDean & Friends Present The Inaugural Dean of Defense "DOD" Youth Camp in the Philadelphia/South Jersey Area on Saturday, June 1st from 10 AM to 2 PM at @totalturfexperience, 614 Lambs Road, Pitman, NJ 08071. Registration opens on Saturday, April 20, 2024. This free skills camp is open to youth ages 7 to 14.

PARENTAL CONSENT/MEDICAL RELEASE FORM

As the parent/legal guardian of the child(ren) I am signing up, I allow my child(ren) to participate in any activities at DOD Youth Camp. I understand that participation in DOD Youth Camp’s activities may result in serious injuries or bodily harm up to and including death and that protective equipment may not prevent all injuries to players.
I agree to waive, release, absolve, indemnify, and hold harmless DOD Youth Camp, Nakobe Dean, The Brand Architects, Total Turf Experience, and each and all of their officers, members, sponsors, participants, representatives, teammates, coaches, and activities for any claim arising out of any injury to my child(ren) however caused, during any activities at the DOD Youth Camp.
I also certify that my child(ren) has received a physical examination within the last month by a licensed physician and is physically capable of participating in DOD Youth Camp.
I give my consent and authorization to any coaches, assistant coaches, trainers, and/or organizers of the football camp to use their judgment in obtaining medical service(s) for my child(ren) and to act as my surrogate until I can be contacted in the event of an accident, injury, sickness, etc. Permission is also given to an athletic trainer and/or doctor of medicine or dentistry to render and provide my child(ren) with the necessary medical assistance and treatment. This care may be given under whatever conditions are needed to preserve the life, limb, or well-being of my child(ren).
I agree that my child(ren) will abide by all camp rules and acknowledge that the camp organizers may use and/or allow used photos or other media including video for promotional purposes on any social media platforms. I further agree to be responsible for picking my child(ten) up at the facility immediately following the football camp. The camp organizers will not be available to watch your child(ren) after the camp ends at 2:00 pm.
By signing my child(ren) up for DOD Youth Camp, I agree to all the terms and conditions outlined above.

Yes. The camp is free however registration is required for each attendee.
Yes. Additionally, every attendee will need a consent/medical release form signed by a parent or guardian to participate in the camp.
Attendees should wear comfortable athletic clothing suitable for physical activity.
No. You don't have to wear football cleats to attend. You are free to wear sneakers.
No. This camp will not require any football equipment to participate.
