"Awaken" Elevate Summer Camp 2018 | 7/29–Fri 8/3

• Early Bird Registration Rate only $350 until April 8
• Standard Rate: $400 paid between April 9-June 17, 2018
• Late Rate: $450 paid after June 17, 2018.

Deadline to Register: July 11, 2018

After submitting your registration, print this Activities Release & Waiver form and fill it out for the Elevate Youth office. This printed form is required to complete your registration in order to attend Summer Camp.

Be sure to check out the Packing List for what you need to take, and what you should leave at home!

Summer Camp DEPARTURE: Drop off your child at the Youth Center (597 Naugatuck Ave.) Sunday, July 29 at 12:15pm SHARP!

* Please feed them lunch before arriving, or pack a lunch to eat on the bus.

* If you haven’t signed an Activities Release and Waiver Form for your child please do so as soon as you check in your child at registration. It is very important to sign the waiver or your child will not participate in high and low ropes activities.
