• The focus for the day will be on Using Collaboration to Drive Out Waste in the Cost of Care

All SIM PCMH practices, managing organizations and community partners are invited to an in-person Care Coordination Collaborative event to be held July 30, 2019 from 10:00 to 3:30 at the Amway Grand Plaza Hotel in Grand Rapids. A continental breakfast and lunch will be provided. The event will feature practical, take-home tools on topics of importance to all organizations that facilitate and promote care coordination. The Care Coordination Planning Group has identified key themes drawn from participant input for session topics.
