Accelerating the Movement to Value-based Care through Clinical Practice Transformation Friday November 9, 2018 9:00AM – 3:00PM EST Capital Hilton 1001 16th St. NW Washington, DC
 
Purpose:
Showcase large-scale transformation in primary and specialty practice settings emerging from the Transforming Clinical Practice Initiative (TCPI) and how the Centers for Medicare & Medicaid Services (CMS) envisions this accelerating movement to value-based purchasing. Engage professional associations in a dialogue about how to support transformed practices and how to spread their successes within their professional sectors.
 
Objectives:
(1) Orient health professional associations to the movement toward value-based purchasing and how the TCPI transformation model has advanced this work.
(2) Introduce health professional associations to the TCPI transformation model, including the TCPI Change Package and the Power PACs, and solicit feedback from the associations on the Power PACs.
(3) Showcase the emerging exemplary practices and generate a discussion among the SANs and other professional associations about how these health professional member organizations and other influencers can support these practices in sustaining their transformation and spread their successes throughout their sectors.
