"From Innovators to Early Adopters: A Closer Look at Bridging Health Care and Population Health"

Given the growing evidence that nonmedical determinants (such as housing, access to affordable healthy foods, and opportunities for physical activity) have a disproportionate impact on a population’s health, the health care sector has begun to respond with a wide range of activities to bridge health care and population health activities.

The New York State Health Foundation (NYSHealth) is sponsoring the third annual population health summit, cohosted by The New York Academy of Medicine (NYAM) and New York University (NYU), to delve deeper into ongoing efforts in New York State and nationally that demonstrate effective collaboration between health care and other sectors to advance population health.


The summit will feature a diverse set of national and New York State leaders who will share details of their work, how they are tracking progress, lessons learned, and opportunities to replicate efforts in health systems and primary care settings. New York State and City officials will also provide updates of statewide and citywide efforts to help advance collaborations between health care systems and other sectors.


Health care providers, insurers, business leaders, policymakers, public health and community leaders, and researchers should attend this conference.
