Payment Plan REGISTRATION ONLY - Rick Braun's New Year's Eve - Blue Chicago

IMPORTANT INFORMATION

Rules You Must Agree To In Order To Register:

1. An invoice will be sent to me, via Square Merchant Services, on the last day of the month for the following month's amount due (for example, I will receive an invoice on October 30 for the November payment) for two consecutive months, November through December.

2. That invoice is due to be submitted for payment by NO LATER than the 15th of the month (for example, November 15th from the invoice example above)

3. My credit card will be charged by SwanRise Productions when I pay that invoice. This charge is for the payment plan for Rick Braun's New Year's Eve Gala ONLY.

4. If I want to include tickets to the Will Donato show on 12/30/15, I will use the link provided with the confirmation email sent with this registration and will pay immediately and in full the $25 per general admission ticket plus processing fee for that event.

5. If I cannot pay the invoice by the 15th of the month due to vacation or another circumstance, I must contact SwanRise Productions at sandy@swanrise.com or 520 219 3381 NO LATER than the 10th of the applicable payment month. All payments for December must be paid on December 15th.

6. If I do not make a payment by the 15th, I understand that all payments made to date will NOT be refundable and my held tickets will go back on sale to the general public.

7. Tickets reserved in the payment plan cannot be canceled and are non refundable. They may, however, be used by someone else if you notify SwanRise Productions.

8. If I cancel my registration prior to the first payment, a fee of $100 will be assessed and I will be responsible for paying that amount.

9. When all payments are completed, SwanRise Productions will send your ticket receipt via email that are to be used at Will Call at the event. The link to make your hotel reservation will be sent with your confirmation for the payment plan.

All Payment Plan ticket purchases are subject to an $18 per ticket extra fee for administration.

Please review to pick your top THREE table selections. For green tables, please do not choose tables 5, 6, 7, 8, or 33 as they are already reserved. For blue tables, please do not choose 23, 24, 50, 51, 56, or 70. These are reserved for sponsors, etc.

The best seats at that table that are available will be reserved for you once we notify you of your payment plan registration assignment.
