These risks are not only inherent to participants, but are also present for volunteers.I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.I understand while participating in this activity, I may be photographed, filmed or quoted. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns.The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.I grant permission for my image and actions to be photographed, filmed, recorded and displayed in any and every way that Hearts of Fire International Ministries and its ministry affiliates choose to use it.(C) I agree that any purchase, donation, or financial commitment that I make with Hearts of Fire International Ministries, is of my own free will, and is not subject to a refund. I acknowledge that ALL financial transactions are final at my own expense. (D) I grant permission to Hearts of Fire International Ministries and their affiliated associates the right to record and use my image, voice for any purpose deemed necessary by Hearts of Fire International Ministries and their affiliated associates. I waive my right to object to their decisions regarding my image or voice that is filmed or recorded in any way. I waive my right to compensation or legal action of all kinds.(E) I acknowledge that I will not cause a disturbance, commit any violent or abusive actions, nor will I use derogatory language. I agree to be removed if I violated this agreement and will not hold Hearts of Fire International Ministries, their directors, officers, volunteers, representatives, and agents responsible for what it will take to remove me. I acknowledge that no weapons are allowed.BY PURCHASING THIS TICKET OR ATTENDING THIS EVENT I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT.
