Ontario CERT Basic Academy

This is the registration form for the Community Emergency Response Team (CERT) Basic Academy in the City of Ontario.

The class is FREE and open to anyone interested in being better prepared for emergencies or disasters. Please complete one registration form per individual (NOT HOUSEHOLD). Minors 16 or older will be required to obtain parent/guardian approval prior to taking the course. You must complete all of the information, including supplemental fields, for your registration to be accepted.

The class will be taught in multiple sessions:

Tuesday, April 7 from 6:00pm-9:00pm

Tuesday, April 14 from 6:00pm-9:00pm

Thursday, April 16 from 6:00pm-9:00pm

Tuesday, April 21 from 6:00pm-9:00pm

Thursday, April 23 from 6:00pm-9:00pm

Tuesday, April 28 from 6:00pm-9:00pm

Thursday, April 30 from 6:00pm-9:00pm

It is required to attend each training session in order to graduate. Participants must be present for the full duration of the class.
