<DIV CLASS="_1mf _1mj"><SPAN>Together you will create a stained, painted, distressed, stenciled, and personalized 18" x 14.5" Minnesota wood sign! All materials and a pre cut Minnesota will be provided for you. You will be sanding and attaching hardware to hang the Minnesota.</SPAN> </DIV>

<DIV CLASS="_1mf _1mj"><SPAN>You will pick one of the three design options </SPAN></DIV>
<DIV CLASS="_1mf _1mj">1.) Coordinates of your humble abode. </DIV>
<DIV CLASS="_1mf _1mj">2.) The first letter of your last name with your last name overlapping the lower half of the letter.</DIV>
<DIV CLASS="_1mf _1mj"><SPAN>3.) "We may not have it all together, but together we have it all." quote.</SPAN></DIV>

<P><STRONG>Safety Waiver</STRONG></P>
<P>I understand that I will use and/or be exposed to drills, sand paper, wire, paint and oil based stains during the Creative Gathering Boutique workshops.  I agree to be responsible for any damages to Creative Gathering Boutique's tools, equipment or facilities, to the extent and in the percentage I cause such damages.</P>
<P>I recognize there may be certain inherent risks in participating in Creative Gathering Boutique's workshops.  I agree to take precautions and observe all Creative Gathering Boutique's policies, recommendations, directions, rules and warnings; whether provided orally or in writing. I assume full responsibility for personal injury to myself.</P>
<P>I further agree to release and discharge Creative Gathering Boutique for any injury, loss or damage that may arise out of my participation in a Creative Gathering Boutique workshop or my presence in the Creative Gathering Boutique facility whether caused by me, Creative Gathering Boutique or any third party.</P>
<P>I agree to indemnify and defend Creative Gathering Boutique against all claims, causes of action, damages, judgments costs or expenses including reasonable attorney’s fees and other litigation costs which may in any way arise from my participation in Creative Gathering Boutique workshops or my presence in Creative Gathering Boutique facilities.</P>
