Do I need to complete a CRNT01 Form?

Yes, you need to complete the form prior attending and have it approved. If you are not a DAC employee, check with your supervisor if any authorization form is required.

Would I receive credit hours?

Yes, an OSDT form would be at the registration table, and a certificate would be sent through upon completion for those that aren't DAC employees. It is an 8 hour training day.
