Eskenazi Health has a belief that in order to improve patient care it is important for employees to focus on self-care. It partnered with the Center for Mind-Body Medicine to develop a self-care program for its employees focused on nutrition, physical activity, mindfulness and stress reduction to help them take better care of themselves and incorporate healthy activities into their daily lives.

Agenda

1 – 1:15 p.m. Welcome and introductions

1:15 – 2 p.m. Reinventing Health Care, Dr. Lisa Harris, CEO, Eskenazi Health

2 – 2:45 p.m. Improving In-Home Outcomes while Cutting Costs
Dr. Charles Miramonti, chief of Indianapolis EMS and chief of care integration at Eskenazi Health
Dr. Miramonti will discuss Eskenazi’s work in using novel in-home providers in combination with population health-based care plan management to improve outcomes and cut costs.

2:45 – 3:15 p.m. Networking & Refreshments
This allotted time will be available for attendees to meet with peers and discuss population health at their respective organizations.

3:15 – 4 p.m. Hospital Tour
A tour of the key features of Eskenazi Health pertaining to population health will be given to those who choose to stay following the networking period.

Dr. Lisa Harris, CEO of Eskenazi Health, has dedicated her career to improving health care for vulnerable and underserved populations and she will present on reinventing health care. Dr. Charles Miramonti, chief of Indianapolis EMS and chief of care integration at Eskenazi Health, will discuss Eskenazi's work in using novel in-home providers in combination with population health-based care plan management to improve outcomes and cut costs. There will be time allotted for a tour of the key features of Eskenazi Health as well as time scheduled for networking.
