- Late arrivals will not be admitted and will be asked to reschedule
- We suggest arriving 15 minutes early with your photo ID to allow enough time to check in

PLEASE NOTE
- Testing is by appointment only – do not bring any guests with you
- Photo ID is required – it must be current
-Your Social security number or passport number will be required to receive AFH certificate.
- Testing time will be 2 hours
o This includes the time it takes to fill in the information at the top of the answer form
o Those who are unable to complete the test in 2 hours can submit the completed portions and return at the next offered session
- Tests will be supervised for the entire two hours
- Providers must pass the exam within 2 tries
- Substitute Caregivers can try to take the test repeatedly
o There is no limit to how many times a caregiver can take the test but it is best to have your caregivers educated on the material before hand
- This is a one-time certification
o Keep your certificate forever
o We do not keep a copy of it
- Interpreters are not allowed
o Staff are required to be literate in English both oral and written communication
- No assistance from others can be provided during the test
- Phones, bags, notebooks, snacks etc…. will be stowed on a table away from the test-taker during the test.
