Join us for an engaging virtual event hosted by the National Association of Health Services Executives (N.A.H.S.E), featuring health equity leaders from Novant Health. In this virtual event, esteemed health equity leaders from Novant Health will engage in a dynamic conversation focusing on a multi-faceted approach to addressing health disparities within healthcare systems. The discussion will delve into the crucial role of data utilization, gap closure initiatives, and the implementation of Healthy People 2030 objectives. Moreover, the significance of gathering comprehensive data, including factors such as race, ethnicity, language, gender, age, and socioeconomic status, will be highlighted in understanding and addressing social determinants of health.

Objectives:

1. Leveraging Data for Action: Exploring strategies for utilizing data effectively to identify and address disparities in healthcare access and outcomes. Discussions will encompass the continuum of care, from acute care settings to outpatient services, and outline actionable steps when individuals screen positive for health disparities. There will also be an emphasis on the importance of gathering comprehensive data to understand patients' social needs and tailoring interventions to address them effectively. This includes an examination of social determinants of health and their impact on health outcomes.


2. Bridging Gaps: Highlighting initiatives such as the Mammography Equity for All initiative and discussing their impact on closing gaps in healthcare access and outcomes, particularly in underserved communities.


3. Healthy People 2030: Exploring the role of Healthy People 2030 objectives in guiding health equity efforts and promoting health for all individuals across diverse populations.
