About this event

Through the Wisconsin Family Connections Center, a short-term case management program (GIFTS – Giving Incredible Families Tools for Success) is available to those who meet the eligibility requirements. The GIFTS Program is designed for caregivers with adoption or out-of-home care experience (this includes reunified, post-adoptive, or post-guardianship families) who are identified as having a high need for services. Families may self-refer or be referred by a community service member.

Join the Wisconsin Family Connections Center for an information session on GIFTS case management program. This session will provide an overview of the program, outline eligibility requirements, and provide insight into services that could be offered. Please note this will be an informational session and not a specific case consult, but there will opportunity for Q&A.

Sessions will be facilitated by GIFTS Case Managers. Please note: This is not a series. Please ensure you have chosen the date/time selection you are interested in prior to selecting “Register.” For more information https://wifamilyconnectionscenter.org/help-me-find/short-term-case-management/

The Wisconsin Family Connections Center (WiFCC) is funded by the Wisconsin Department of Children and Families. The WiFCC provides support and services to parents and caregivers across the continuum of kinship care, foster care, and post-post permanency, including reunification, guardianship, and adoption. The WiFCC also serves as an excellent resource for professionals serving these families and caregivers.
