If you wish to complete a healthcare directive, you should prepare the following information in advance:
- Who will be your healthcare agent(s)? Your healthcare agent is the person you would like to make healthcare decisions for you if you are no longer able to do so. You may select a primary healthcare agent and backup(s) or you may designate multiple healthcare agents that must all act together.
- What are the address and phone number of your healthcare agent(s)?
- Do you have any specific wishes or instructions that you would like your healthcare agent(s) to be aware of?
If you wish to complete a power of attorney, you should prepare the following information in advance:
- Who will be your attorney-in-fact? The attorney-in-fact need not be an attorney, but should be someone that you trust to make financial decisions for you (e.g., property transactions, banking transactions, etc). The power of attorney document is valid even while you are capable of making decisions yourself, so make sure your attorney-in-fact is trustworthy.
- Do you want to appoint more than one attorney-in-fact? You can require them to make decisions together or appoint one primary attorney-in-fact and backup(s).
- What is your attorney(s)-in-fact's address?
