CYP Continuing Care Training

These events are open to staff from Health, Social Care and Education. An overview will be provided of Continuing Care including the processes of referral process It will provide professionals with the necessary knowledge to complete a pre assessment referral, fast track referral and prepare them to fully participate in continuing care assessments and reviews in a confident manner. The commissioning process of procuring packages of care and personal health budgets will be discussed. Target Audience: Professionals who work and support children and families who have complex health needs.
