Conducting Effective Incident Investigations (and how sometimes we get it wrong)
Program Description:
An effective incident investigation program is an essential element of an organization's safety management systems. However, some precautions must be kept in mind. Incident investigations are only one component of a health and safety program and are ultimately a reactive process. If we only learn when things don’t go well, then we will always be limited on how our business improves and adapts to the operational challenges of any industry. How do we truly learn from these events, and why do we have to limit ourselves to learn only when something bad happens?
