Colorado Health Navigator Assessment - May & June

**IMPORTANT NOTES**

In-person Dates: 5/17 AM, 6/4 AM, and 6/11 AM

Virtual Dates: 5/18 AM, 6/6 AM, and 6/14 AM

PLEASE REGISTER FOR THE DATE THAT ALIGNS WITH YOUR PREFERRED TESTING MODALITY (IN PERSON OR VIRTUAL)

The cost of this assessment is $363 per participant

plus ticket vendor payment processing fees

The fees are as follows:

$10.89 - check payment fee ($373.89 total)

$20.96- credit card payment fee ($381.33 total)

Please select ONE of the time slots to register for the Colorado Health Navigator Assessment,

IN PERSON at CAPE: 1890 N. Revere Ct, Aurora, CO 80045 (CAPE is located on the 3rd floor of the Anschutz Health Sciences Building)

Registration Timeline:

Registration is open.

**PLEASE NOTE: Preference for payment is by debit or credit card to ease tracking and secure your registration. If you select "pay by check" the check must be received TWO weeks prior to the assessment.

**CHECK PAYMENTS: If you are paying for multiple participants by check, please send in a separate check for each participant and identify the names of the participants and should be written out to "Regents of the University of Colorado". Checks can be mailed to: Elshimaa Basha, 1890 N. Revere Ct., Mail Stop F495, Aurora, CO 80045.

If all slots are filled, you may add yourself to the waitlist by selecting the waitlist option. Please leave your name, contact information, and desired time slot. You will be contacted once a slot becomes available.

Cancellation Deadlines:

30 days prior to the assessment date: 50% cancellation fee.

After 30 days: 100% cancellation fee.

Please note: no shows will be charged the full assessment fee as well.
