At this meeting, members will identify strategies to screen patients in primary care for a growing list of conditions and needs (including medical, mental and social needs), learn about improving processes to act on positive screening results, and hear about member progress identifying and addressing social determinants of health. Ambulatory care leaders and PRIME operational leads are encouraged to attend, but to support small group discussion please coordinate for no more than four attendees from each member system. 
While we are looking for four attendees per member system, SNI can reimburse travel expenses for up to three people from each system; our travel policy is <A HREF="https://safetynetinstitute.org/wp-content/uploads/2019/03/caph-sni-non-stf-expense-report-2019-care-delivery-workshop.xlsx" TARGET="_blank" DATA-MSYS-CLICKTRACK="0" REL="nofollow noopener noreferrer">attached</A>.
 Each system attending will be asked to complete a survey regarding screening in primary care. The survey will be sent by March 27 and will take about 15-20 minutes. We are looking for a single response from each individual system by April 12.
