About the Event
Chronic illnesses and behavioral health disorders are closely linked but often treated in isolation. Individuals with chronic conditions are significantly more likely to experience mental health challenges, while those with serious mental illness face reduced life expectancy due to preventable physical complications.
Breaking Silos brings together experts to explore integrated, person-centered care models through keynotes and panel discussions. Attendees will learn about evidence-based strategies, policy developments, and community interventions that address the full spectrum of health—both physical and mental.
