Hospital programs that connect patients with case management and treatment for psychological trauma can reduce cycles of community violence.

Program Description: Violence is an epidemic, disproportionately hurting young people of color. Victims of violence are at increased risk for repeated injury and poor health. Hospital programs that connect patients with intensive case management and treatment for psychological trauma can reduce cycles of community violence. The intent of this program is to provide strategies to improve practice in hospital – linked violence intervention strategies.

Target Audience: This program is appropriate for health care providers, social workers, behavioral health providers, first responders and anyone who is interested to decrease violence in the community. 3.75 units Continuing Ed / CME available

Objectives:

1. Identify best practices in secondary prevention in the hospital setting.

2. Utilize community resources in the post hospital period to enhance trauma informed care to reduce reinjury, incarceration, and death

3. Develop an interprofessional network to reduce the incidence and impact of violence in Delaware County
