Amanda-Clearcreek High School and Fairfield Medical Center are offering pre-participation physical exams. Only offered to athletes playing in the Amanda-Clearcreek school district.
The AC Coaches Club is accepting $10 donations the day of the event to continue supporting athletic programs.
Please book one appointment slot per athlete and include the appropriate name of the athlete booked for that time slot.
Please complete the 2024-2025 pre-participation physical evaluation form from OHSAA, which is found at www.ohsaa.org. This document must be signed and completed by the parent prior to the scheduled appointment. Athletes 15 and younger will need to be accompanied by an adult during the exam.
