Join the Michigan Pharmacists Association, University of Michigan College of Pharmacy, Michigan Health Information Network (MiHIN), and leaders from across population health, pharmacy, government, payers, and more on January 24, 2024, from 10 a.m. - 3 p.m. for an in-person Advanced Medication Reconciliation Workshop that will delve into a statewide strategy for implementing a two-pronged technical framework to optimize medication use safety and outcomes.

One of the most challenging aspects of improving medication use safety and outcomes is to ensure that accurate information about a patient’s medication use is available across the healthcare continuum.

This workshop aims to capture the dimensions of the status quo, foster discussion of perspectives, reveal ideal state consensus objectives, identify constraints or barriers, and review the utility of existing statewide use cases and infrastructure.

Participants will collaborate to frame current challenges and identify and prioritize action steps to begin testing a statewide data-sharing use case and related care coordination infrastructure through targeted pilot activities; these pilots will lead to the establishment of protocols and critical statewide infrastructure to navigate the technical, financial, and legal and organizational dimensions necessary to improve medication reconciliation.
