Advance Care Planning Presentation/Workshop

My Care, My Plan’s vision is for every adult 18 and older in Sonoma County to become educated and empowered to express his/her wishes about end-of-life care, to have the opportunity to do so and to have his/her wishes honored.We want to help people in our community contemplate, communicate about and properly document the kind of medical care they want before a medical crisis occurs or they face a serious illness. We want people to document those wishes in writing and have conversations with their health care agents, loved ones and doctors.

Get tips and resources on how to select a healthcare agent to speak on your behalf should you become unable to speak for yourself. Find out how to complete and update your advance health care directive.
The first half of this session will focus on advance care planning. The second half will focus on how to complete an advance health care directive.
