Long Island Population Health Summit

The Long Island Health Collaborative is a partnership of Long Island’s hospitals, county health departments, physicians, health providers, community-based social associations,  human service organizations, academic institutions, health plans, local government, the business sector, and more. The LIHC is the hub of population health activities on Long Island, and works together to provide a healthier community for the people we serve using the expertise and resources of our many unique members.*Population health activities of the Long Island Health Collaborative are funded by the Long Island Health Population Health Improvement Program, a New York State Department of Health grant.

<h2>This is not the value-based purchasing presentation that we have all been to!</h2>What is it?The purpose of this event is to offer participants an inside look at a working value-based purchasing (VBP) arrangement, noting the interdependence of these contracts and the social determinants (SDH) of health.Who should attend?Anyone who has an interest in ensuring that affordable, quality healthcare is accessible to all Long Islanders.This includes small to large-sized businesses that purchase insurance, and who may also be interested in developing VBP arrangements with their insurers, to healthcare and medical suppliers and manufacturers, to anyone working in healthcare and social services and public health – community-based organizations, hospitals, health departments, health plans, etc.Why should you attend?Hear from keynote speaker, Ryan P. Ashe, Director of VBP at the NYS Department of Health, be the first to receive new data from the LIHC, and learn through a unique panel of the involved parties of a currently working VBP arrangement.Don't miss our unique panel which will feature the key participants of a healthcare housing pilot project happening now on LI.The panel will include the involved CBO, primary care provider, and health plan. Panelists will tell their VBP story, including successes, obstacles, and tips for others in a candid discussion.Plus: Academic partners who support our workforce pipeline and master's-level students will be present to discuss possible opportunities for assistance with your organization's special projects.The summit will take place in the Health, Sports and Education Center on SCCC's Michael J. Grant Campus.
