The Long Island Health Collaborative is a partnership of Long Island’s hospitals, county health departments, physicians, health providers, community-based social associations,  human service organizations, academic institutions, health plans, local government, the business sector, and more. The LIHC is the hub of population health activities on Long Island, and works together to provide a healthier community for the people we serve using the expertise and resources of our many unique members.*Population health activities of the Long Island Health Collaborative are funded by the Long Island Health Population Health Improvement Program, a New York State Department of Health grant.

Sunken Meadow State Park

Join the Long Island Health Collaborative, AARP Long Island, Stony Brook Medicine, the Suffolk County Office of Minority Health, and the Long Island Sound Study for a very special installment of Walk with a Doc at Sunken Meadow State Park, located right on the estuary that is the Long Island Sound.Walkers meet by main bath house, park in Field 1. Park is accessible to those in wheelchairs and or those with mobility issues. <ul><li>Dr. Anupama Paranandi of Stony Brook Medicine will discuss how we can prevent and manage chronic diseases as we age.</li><li>Adesuwa Watson, Director of Suffolk County DOH’s Office of Minority Health will share insight into the OMH’s efforts to align with the Long Island Sound Study to improve equity in the region and access to the Long Island Sound for all</li><li>Jimena Perez-Viscasillas, Long Island Sound Study Outreach Coordinator will speak about the New York Sea Grant, the Long Island Sound Study/Long Island Sound Estuary Program, and environmental health</li></ul>After hearing from Dr. Parandi, Ms. Watson, and Ms. Perez-Viscasillas, they will join attendees on a walk through beautiful Sunken Meadow Park.More About Our 2024 Walk with a Doc SeriesAre you over 50?
