Pre-participation physicals exams at Center High School provided by Sports Medicine Center at Children's Mercy physicians.

Center High School

Appointments are required. No walk-ins will be accepted day of the event. Sign-ups will close one week prior, on 7/4. Day of: <ul><li>Attend at your designated appointment time.</li><li>Enter high school on east side through doors across from the soccer fields. </li><li>Please have all pre-performance physical evaluation paperwork completed prior to your appointment time, including all parent and student-athlete signatures. </li><li>Set up your PRIVIT account prior to physical date so that your physical can be uploaded immediately after completion.</li><li> Wear comfortable or athletic clothing that is easy to move in.</li></ul>When selecting ticket "time", only select 1 per athlete. If you change the time you would like you will need to cancel your original order and then select a new ticket. For "Attendee" please enter the student-athlete's name that will be attending during that time. Only 1 athlete per ticket. If you are signing up for 2 different student-athletes you will need to select separate tickets. -Thank you!
