It is impossible to determine who has it, and who does not given the current limits in virus testing. I knowingly and willing consent to participate in the 19th Annual Jon Hastings Memorial Tournament presented by the Tennessee Wireless Association during/post the COVID-19 pandemic. I confirm that I will not attend the golf tournament if I am presenting any of the following symptoms of COVID-19 listed below:

Loss of Sense of Taste or Smell

Dry Cough

Runny Nose

Sore Throat
