A conversation helping families understand how home care, assisted living, and long-term care work together to support aging loved ones
Join Us for the Care Continuum Roundtable Talk!
Navigating care for an aging loved one can feel overwhelming—especially when you’re trying to understand what kind of support is needed, when it’s needed, and how to plan for what may come next.
On Tuesday, September 22 at 4:00 PM, we’re bringing together professionals from Griswold, Summit Place, and CareScout for a Care Continuum Roundtable Talk at Summit Place in Eden Prairie. Rather than three separate presentations, this will be an interactive, scenario-based conversation focused on real situations families face every day. We’ll talk through how home care, assisted living, and long-term care planning can work individually—and, more importantly, how they can work together as someone’s needs change.
We’ll tackle questions like: When is staying at home still realistic? Does moving to assisted living mean giving up independence? What does long-term care insurance actually cover? Why might someone in assisted living still benefit from home care? And how can families start planning before they’re forced to make decisions during a crisis?
Meet the Sponsors & Panelists
Griswold Care – Home Care
Griswold provides non-medical home care designed to help older adults remain safe, supported, and independent wherever they call home. Our caregivers can provide companionship, personal care, homemaking, transportation, respite support, and additional one-on-one assistance as needs change.
During the conversation, we’ll help families understand what home care actually looks like, when bringing additional support into the home may be enough, and how home care can complement assisted living and other services. Most importantly, we want families to know what resources are available before they find themselves scrambling during a crisis.
Summit Place – Independent Living, Assisted Living & Memory Care
Summit Place is a vibrant senior living community offering Independent Living, Assisted Living, and Memory Care, all on one campus. Their goal is to provide the right level of support while helping residents continue to live life on their terms.
Summit Place will help take some of the mystery out of Assisted Living—from recognizing when it may be the right choice and understanding the transition process to what everyday life actually looks like. We’ll also discuss how families can prepare emotionally, practically, and financially, the services and opportunities available to residents, and how long-term care insurance may fit into the process.
CareScout – Long-Term Care Navigation & Funding
CareScout helps older adults and their families navigate the aging journey, find quality care, and understand how to fund that care. With more than 20 years of experience helping families navigate aging care, their goal is to make an often complicated system easier to understand.
CareScout will share how long-term care insurance can be a powerful tool when families are searching for and funding care, when to start reviewing a policy, and how understanding your benefits before you need them can help you make more informed decisions.
The goal of this event isn't to tell families there is one "right" answer. It's to give you practical information, different perspectives, and a better understanding of the options available so you can feel more prepared when care needs begin to change.
