Neighbourhood Health and Connecting Care for Children

Brief Overview:

This session will bring together national policy, practice and cultural leadership. This session will use CC4C as an example of neighbourhood health for children in action. The question of what role kindness plays in making integration work and the influence on patient and population outcomes will be explored.

• Identify how CC4Cs model reflects Neighbourhood Health aims in practice
• Consider the role of kindness in delivering neighbourhood health effectively
