Malverne Tennis Program:

Tennis Training - Saturday Program for Youth & Adults

• 6 weeks beginning Saturday Sept. 19th
• Ages 6 and up
• Incorporated Village of Malverne Residents ONLY
• 1 hr lesson - 6 weeks - $100
• 8 participants per hour
• Sat 8AM-12PM and Sun 8AM-11AM

PLEASE NOTE: Due to the commitment to the trainer, refunds will only be issued prior to the start of the FIRST class. There will be a $25 cancellation fee if you decide to cancel your class.

• Bring your own racket. Balls will be provided.
• Please do not join a class other than your own.
• All classes will be held on the tennis courts behind Malverne High School.
• Please arrive 10 minutes before your scheduled time to stretch without supervision.

By purchasing Malverne Tennis Training lessons, you give permission for your son/daughter/self to participate in the Malverne Tennis Program. You also understand that if you decide not to participate in this class, full refunds will only be issued prior to the start of the FIRST class. There will be a $25 cancellation fee if you wish to cancel the class after the FIRST class only.

Malverne Tennis is a fair-weather program. If the courts are wet, they become slippery and are a hazard to the participants. In the event of rain and/or wet courts, lessons will be postponed until the following weekend since the instructor will not wait around for the courts to dry.

Participation Waiver:

By purchasing Tennis Lessons, I hereby acknowledge that there are certain risks in participating in The Incorporated Village of Malverne ("Village of Malverne") Tennis Program held at Malverne High School ("MHS") facilities at 80 Ocean Avenue, Malverne, NY 11565 (the “Activity”). In consideration of the Village of Malverne allowing myself and/or my child to use its facilities to participate in the Activity, I hereby assume all risks associated with the Activity, including but not limited to, full and complete responsibility for any injury or accident which may occur to myself and/or my child in connection with the Activity. I knowingly and intentionally hereby release and waive any and all claims, of whatsoever kind or nature that I may have against Village of Malverne, its Board of Trustees, employees, agents and representatives, resulting in whole or in part, from participation in the Activity. This release and waiver shall also be binding on my heirs, administrators, and assigns.

In this regard, I indemnify, hold harmless, release and discharge the Village of Malverne, its Board of Trustees, agents, employees, and officers, from any and all claims, demands, liabilities, actions, judgments, expenses (including attorneys’ fees and costs of defense), and executions which may be made by reason of any injury to myself and/or my child (including, but not limited to, serious bodily injury or death), or damage to property sustained by myself and/or my child, caused by any act, neglect, default, or omission of any person, firm, or corporation, directly or indirectly associated with the Village, arising directly or indirectly out of participation in, or association with the Activity.

I HAVE READ AND UNDERSTAND THIS AGREEMENT AND I AM AWARE THAT BY PURCHASING LESSONS, I AM WAIVING CERTAIN SUBSTANTIAL LEGAL RIGHTS WHICH I AND MY HEIRS, PERSONAL REPRESENTATIVES AND ASSIGNS, MAY HAVE AGAINST THE VILLAGE.

Photo Release:

I hereby grant the Village of Malverne permission to use photograph, video, or other digital media (“photo”) that may be taken of me or my child as a Participant in the Activity in any of its publications, including web-based publications. I acknowledge that the images may be used, without limitations, in print publications, online publications, websites and on social media. I understand and agree that all photos will become the property of the Village of Malverne and will not be returned.

I understand that the images may be modified in any manner, and I hereby waive any right to inspect or approve any final product that uses my photographs. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge the Village of Malverne from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

I HAVE READ AND UNDERSTAND THE ABOVE PHOTO RELEASE. I AFFIRM THAT I AM AT LEAST 18 YEARS OF AGE, OR, IF I AM UNDER 18 YEARS OF AGE, I HAVE OBTAINED THE REQUIRED CONSENT OF MY PARENTS/GUARDIANS AS EVIDENCED BY THEIR PURCHASING TENNIS LESSONS.
