It aligns the people who deliver care with the people who live with its outcomes. Traditional managed care treats providers like vendors and individuals like compliance problems. The result is predictable. Fragmentation. Workforce collapse. Shallow value-based contracts. No one with real accountability at the table when decisions are made.</p><p>The Community Supported Care model replaces that architecture with a shared one.
