REQUIREMENTS

Believe it or not, you don't need a certain degree to be a care manager.

As a care manager, you join with families to assess needs, create a care plan, and coordinate support for an aging or disabled loved one. You guide families through stressful moments, offer education, and help them make thoughtful, informed decisions.

The only requisite is to have experience working with older adults/disabled/cognitively impaired or IDD clients and their families.

Best practices in care management will be learned AND shared in this collaborative environment.

  • Home Care employees
  • Physical therapists
  • Occupational therapists
  • Speech therapists
  • Social workers
  • RCFE and SNF administrators
