This COVID-19 vaccination clinic hosted by <a href="https://www.hillsdalehospital.com/resources/covidvaccines/" target="_blank" data-msys-clicktrack="0" rel="nofollow noopener noreferrer">Hillsdale Hospital</a> will be held in the Searle Center at Hillsdale College. Please read this description in its entirety before registering in order to avoid any issues at your vaccination appointments.If you are a parent/guardian who is registering a minor, you will be required to register your child and confirm on the registration that they are within the eligible age range for this vaccination clinic (12 years and older), providing their date of birth. A parent/guardian/adult must be present with the minor during the vaccination clinic. However, understand that if the designated adult is not a parent or guardian, the child will still need a signed parental consent form upon arrival. Below is a list of very important items as you prepare to receive this vaccination:<ul><li>Print and complete the <a href="https://www.hillsdalehospital.com/wp-content/uploads/2021/01/Vaccination-Consent-Form.pdf" target="_blank" data-msys-clicktrack="0" rel="nofollow noopener noreferrer">linked vaccination consent form</a>.</li><li>If you are registering a minor, please also print and complete the<a href="https://www.hillsdalehospital.com/wp-content/uploads/2021/05/parentalconsentform.pdf" target="_blank" data-msys-clicktrack="0" rel="nofollow noopener noreferrer"> linked parental consent form</a>. Parental consent is required for all minors who wish to receive a vaccination.</li><li>All attendants must present a form of ID at the time of check in.</li><li>Plan to spend 30 minutes at the vaccination clinic (we must observe participants for 15-30 minutes after vaccination).</li><li>All attendants must wear a mask at all times and engage social distancing</li><li>Read the <a href="https://www.hillsdalehospital.com/wp-content/uploads/2021/01/Pfizer-BioNTech-COVID-19-Vaccine-EUA-Fact-Sheet-for-Recipients.pdf" target="_blank" data-msys-clicktrack="0" rel="nofollow noopener noreferrer">Pfizer vaccination fact sheet</a> prior to arrival.</li><li>Read the <a href="https://www.hillsdalehospital.com/wp-content/uploads/2021/05/Covid-Vaccine-Infertility-Myth.pdf" target="_blank" data-msys-clicktrack="0" rel="nofollow noopener noreferrer">Newsweek article</a> about vaccine myths prior to arrival. </li></ul>In order to receive a first dose of the vaccine, you or your loved one must be available to receive their second dose of the vaccine on Thursday, October 22, 2021 (time and location to be announced).In order to receive a third dose of the COVID-19 vaccine, you must meet one of the following criteria and will attest to this upon registration and at the vaccine clinic. Criteria for Third Pfizer Shot<ol><li>Receiving active cancer treatment.</li><li>Had an organ transplant and receiving immunosuppressive medication.</li><li>Taking any immunosuppressive medication to suppress immune system.</li><li>65 years of age or older</li><li>18 through 64 years of age whose frequent institutional or occupational exposure to SARS-CoV-2 puts them at high risk of serious complications of COVID-19 including severe COVID-19 (healthcare workers, teachers and daycare staff, grocery workers, first responders)</li></ol>
