Join West Health’s monthly webinar as we share the experiences from our Learning and Action Networks. In partnership with the Institute for Healthcare Improvement and organizations who are actively developing home-based alternatives to care for seniors’ unplanned acute care needs, we will present valuable insights, tips, and overcoming challenges in this emerging field. The overarching aim of the webinar series is to advance the work of those leading organizations who have accepted financial risk for their senior patient population and offer innovative or disruptive home-based alternatives to the traditional treatment for those seniors experiencing unplanned acute events. The Webinar Series will focus on the acute services provided in the home by these innovative organizations and how through their efforts they are working to lower the cost of healthcare. You will hear directly from organizations on how they have created a sustainable business model, how they measure quality and success, optimize the utilization of their home-based acute care programs, and how they are avoiding emergency room visits and hospital admissions. These leading organizations will share data, process steps, challenges/success, and look to guide direct discussion around setting up your own home based programs.
