A representative from the Dept. of Finance will be available to help you. Bring these documents for ALL owners:

• Government-issued photo ID
• 2016 Income documents (i.e. tax returns, SSA, SSA, SSDI, pension & annuity statements, W2, etc.)
• SSA Disability Noticed of Award Letter or other proof of disability
• Trust or Life Estate documents (if applicable)
• Death Certificate (if applicable)
• All owners must be present to sign the application and submit it at the event

Forms available at the time of the event will be distributed.
