(Hampton) PERSON-CENTERED ISP PLAN AND PLANNING PROCESS

Training on Virginia’s new and improved Person-Centered Individual Support Plan and Planning Process is available in March in your region. Space is limited to managers, supervisors, staff trainers and support coordinator supervisors working with individuals who receive Community ID or DD Medicaid services. Staff attending should have the ability to train others in their agency.

This training was developed by Community Resource Consultants from the Division of Developmental Services with the Department of Behavioral Health and Developmental Services. The instructors will provide an in-depth review of the 5 parts of the Virginia ISP and ongoing documentation requirements. In addition, the instructors will facilitate exercises to improve staff skills for developing outcomes and support instructions. The new ISP process should be used for individuals in the ID and DS Waivers with any annual plan meetings held on or after April 1, 2015. DD Waiver providers may also use the new plan, but are not expected to at this time.

Training includes: New elements in the ISP; practice developing meaningful outcomes; learning the differences between supporting and skill-building in documentation; how to write support instructions; reporting progress in person-centered reviews.
