In collaboration with various partners, the Transition and College Access Centers will support schools and families with student success while facilitating a seamless transition for students from school to adult life, in preparation to be productive members of society.  All students will be provided with experiences that will foster their academic, social-emotional and vocational growth and excellence, facilitating multiple pathways to graduation and success after high school.
