Before you're permitted to input Lithuania, you'll need to fill out a Lithuania Registration Form. This is needed by all foreign residents of Lithuania, even though they might not yet have a resident visa. The Ministry of Foreign Affairs in Lithuania will allow you to acquire a visa in order to travel to Lithuania, however you will still have to complete the enrollment form before you may travel. List all of your personal items. These include: passport, vaccination card, airline tickets, and any prescriptions. You'll also have to list all your personal items that are in your luggage or in your vehicle. It is critical to indicate whether any of your prescription medications have been in your possession, including: vitamins, aspirin, as well as blood pressure medications.
List all of your personal illnesses and conditions. You will need to clarify if your illness or illness isn't yet diagnosed. If it isn't a contagious disease that can spread easily, you'll also have to describe if it has a particular title (i.e. strep throat). The only contagious diseases that need an entry into the vaccination center's database are those which are automatically contracted through contact, inhalation, or blood.

It's also advisable to indicate if you were previously divorced or still currently being wed. The health authorities will also need to know the names of any children or individuals who are living with you. Make sure you record the legal names of those individuals as well, for example, city and city they reside in. Each of these details must be in your registration form for Lithuania.
