LifeCircles PACE
LifeCircles helps people 55+ age well in their homes, and avoid nursing facilities whenever possible. We provide customized health and social supports to help people with complex health or memory challenges live as independently as possible. The innovative non-profit organization has been serving West Michigan's Lakeshore since 2009. LifeCircles' team of senior care experts partners with individuals and care givers with compassionate, comprehensive, team based care that honors health care preferences and desires to remain living in the community.
We envision a community where every individual has an opportunity to live in their own home and experience independence, as they define it, while exercising freedom of choice in all matters affecting their lives. We seek to share our knowledge and expertise to help West Michigan become a more inclusive and supportive community.
