In the Lower Level in Conference Room 2. The lights will be on and the door marked with Advance Care Planning signs. Come on in!

What do I need to bring?

Please bring contact information for anyone you may want to name in a legal document who could serve as your trusted medical decision-maker. Bring a photo ID for notary if desired (driver's license or ID card works perfectly). All documents and writing utensils will be provided.

Where are the nearest restrooms?

There are restrooms on the Lower Level of the building, down the hall from the Conference Room, as well as on Levels 1 and 2.

What is Advance Care Planning? An Advance Directive? A Healthcare Agent? A POLST form? You are invited to a workshop on completing an Advance Directive. You will be guided through the process of choosing a trusted decision maker and putting your wishes in writing related to healthcare in the event that you are unable to speak for yourself. All documents will be provided, and a Notary Public will be available to notarize your completed document if desired. Hosted by the PeaceHealth Advance Care Planning Program. Duration of presentation: Approx. 90 minutes, with Q & A at the end. Open to PeaceHealth patients and all members of the community.
