Sheila Kolt, LCSW, CMC
Aging Life Care Professional | Care Management Expert

Sheila Kolt is a seasoned Aging Life Care Professional with over 20 years of experience in care management and clinical social work. She is the co-founder of Concierge Health, a private care management company providing personalized support to clients navigating aging, chronic illness, and life transitions.

Her expertise spans dementia, stroke, Parkinson’s disease, oncology, psychiatric conditions, and more. Sheila has held leadership roles in home care, senior living, and psychiatric care, and has maintained a private psychotherapy practice.

She is President of the New York Chapter of the Aging Life Care Association (ALCA), where she’s been active since 2008, and the Immediate Past President of the Case Management Society of America (NY Chapter). Sheila holds degrees from the University of Pennsylvania and Columbia University, and is bilingual in Spanish.
