"How safe are we? - Feedback from Functional Safety Assessments"

We often think of Functional Safety as being all about reliability, right? But what about systematic failures - are we putting too much trust in reliability and overlooking the risks that come from design or process flaws? Sure, Functional Safety Assessments are mandatory, but they’re also a powerful tool for checking how well we’re adapting to the fast-paced world of automation. In this session, we’ll dive into some of the trends we’ve seen and the common mistakes that tend to crop up, drawing on years of experience assessing systems during both the design phase and throughout their operational life, including modifications.

Functional Safety isn’t just about ticking boxes—it’s about truly understanding how systems behave in the real world. By looking beyond reliability and addressing potential systematic issues, we can build safer, more resilient systems. This session aims to share practical insights and lessons learned to help strengthen our approach to Functional Safety. - Andrew Darbyshire, Technical Partner, ERM
