Below is a list of all required documents needed for your application:
• Valid Driver’s License or Passport
• Affidavit – Must be typed by you; notarization is not required
• Owner’s Statement – Must be typed by you; notarization is not required
• Typed List of Names and Addresses for All Owners/Partners
• Organizational Chart – Must be created by you in a structured format (not a simple list)
• Certificate of Formation
• Business Registration Certificate
• EIN Letter
• Applicable NAICS Codes for your business
