Replacement Diploma Request Form

Types of Requests:

Replacement Diploma (Domestic) - $75 per copy - Estimated 4-6 weeks to receive replacement diploma from vendor once we have received request and payment.

Replacement Diploma (International) - $100 per copy - Estimated 4-6 weeks to receive replacement diploma from vendor once we have received request and payment.

*If you need proof of graduation, you may request a degree verification from the Registrar's Office (registrarsoffice@mcphs.edu) or by calling the office at (617)732-2855.

DISCLAIMER: Diplomas will be printed with the name that we have on record. Unfortunately, name changes will not be granted.
