Passport Application Event with the Office of Congresswoman Suzan DelBene
Event Information:
Time: Thursday, July 24 from 10:00am-3:00pm
Where: Mill Creek Passport Office, 15728 Main St, Mill Creek, WA 98012
Length of appointment: 15 minute time slots.
This Event is for Constituents Who Need to:
Apply for a first time adult passport, first time minor passport, renew a minor's passport, or replace lost/stolen passports.
Information You Will Need to Book an Appointment:
- Full Name
- Contact Information (phone & email)
- Current Address
- Indicating if this appointment is for yourself or a minor
What You Will Need for your First-Time Adult Passport Appointment:
- Please arrive on time to your appointment
- All applicants must appear in-person, even minors
- One current color passport photo: 2” x 2” with plain white or off-white background, free of shadows and no older than 6 months. NOTE: There is a Staples down the street from the Mill Creek Passport Office that can take passport photos
- A certified copy of your marriage certificate or court order if your name has changed
- Birth Certificate (original/certified copy). If you do not have one or need to get a new one, you can do so here. Or an original Certificate of Naturalization or original Certificate of Citizenship.
- Check or money order. See fees below.
- Completed Form DS-11. Fill out and print here.
- A valid Driver’s License or a valid photo ID.
- Please bring photocopies of U.S citizenship (Birth Certificate, Certificate of Naturalization, or Certificate of Citizenship) and photo ID (front and back) to submit with the application.

What You will Need for a Minor's Passport Application (Both Renewal and First-Time Applicants):
Minors (under 16) must apply in person for both first-time passports and renewals. Passport for children under age 16 are only valid for 5 years and need:
- Both parents or legal guardians
- Birth certificate (original/certified copy). If you do not have one or need to get a new one, you can do so here.
- Check or money order
- Expired passport (if available)
- One current color passport photo: 2” x 2” with plain white or off-white background, free of shadows and no older than 6 months. NOTE: There is a Staples down the street from the Mill Creek Passport Office that can take passport photos
- Social security number
- Completed Form DS-11. Fill out and print here.

Please note that adults who need to renew their passport should do so through the mail or online. Directions here. If you feel you need assistance, please make an appointment.
Adult renewals only processed in person if:
- Your most recent passport has been damaged, lost, or stolen, or if it has been expired for more than five years.
- You were under 16 years of age when your most recent passport was issued.

- One current color passport photo: 2” x 2” with plain white or off-white background, free of shadows and no older than 6 months. NOTE: There is a Staples down the street from the Mill Creek Passport Office that can take passport photos

Current Fees
You will need to bring TWO forms of payment with you (two separate checks or money orders). One will go with your passport to the State Department; the other is payable to the City of Mill Creek for processing fees.
Adult Passport Book - $130
Adult Passport Card - $30
Passport Book & Card - $160
Minor (under 16) Passport Book - $100
Minor (under 16) Passport Card - $15
Processing Fee - $35

**This location does not accept debit or credit cards for passport transactions.
Processing Times
According to U.S. Department of State, processing times for routine and expedited applications are as follows:
6-8 weeks for routine processing
2-3 weeks for expedited processing
Mail times vary and delays have been significant. Plan ahead.

Things to Remember
All questions on the application are about the person applying.
- Minors should use someone other than a parent as an emergency contact.
- Social security number is required for all applicants.
- Mother's maiden name is required.
- Spouse's maiden name is required.
- Use black ink only on your application.
- Your emergency contact should not be someone you travel with.
- Visit Travel.State.Gov to learn more.

- If you no longer need an appointment, please make sure to cancel your appointment.
- Appointments may book up quick, if you did not receive an appointment, keep an eye on appointment slots for any cancellations.
