Population Health Learning Network: Session #1

When: Wednesday, May 23, 2018 - Thursday, May 24, 2018

On May 23rd, 2018: Breakfast will be served at 10:00 am and program content will begin at 10:30 am. Lunch will also be served around noon. Program content will end at 5:00 pm followed by a networking reception on-site until 6:30 pm.

On May 24th, 2018: Breakfast will be served at 8:00 am and program content will begin at 8:30 am. Lunch will also be served around noon. Program content will end by 2:00 pm.

Who Should Attend: Each organization should plan to send the core team participating in the PHLN network. Due to the size of the cohort, we are asking that each organization limit their attendance to five team members.

Description: Over our two days together, CCI plans to engage you in a variety of learning and sharing activities, workshops and learning labs with presentations by peers and experts, and discussion groups in order to:

Identify population health management topics most pressing to participants for best practices and lessons learned;
Take a deep dive into the areas of addressing social needs, behavioral health integration, complex care management, alternative visits, becoming a learning organization, and using data, technology, and tools for population health management;
Map potential bright spots for future programming;
Identify specific challenges or technical assistance needed to improve your population health capabilities; and
Make sure you leave the convening with commitments, next steps, and a buddy to report to over the next 60 days.

Parking & Transportation: For a map & directions, click here. Parking at the hotel is $12 with vouchers that will be handed out at the meeting. The hotel is located nearby the Oakland Airport and two miles from the BART system; the closest BART stop is Oakland Coliseum.

Lodging: We have a limited number of reserved rooms available at the Hilton Oakland for the nights of May 22nd and May 23rd at a rate of $200/night, first-come, first-serve.
