Navigating Care Transitions

Smooth, well-coordinated care transitions can mean the difference between stability and crisis for older adults and people with disabilities. This one-hour training will provide practical strategies for healthcare providers, care managers, and caregivers to ensure continuity of care, reduce risks, and support dignity during times of change. Participants will learn best practices in communication, care coordination, medication management, and family engagement—leaving equipped with tools to improve outcomes and quality of life.
